Outpatient Registration

It is our first preference that our patients use our ELECTRONIC MEDICAL RECORDS SYSTEM (CHARM PHR) to submit their medical history, questionnaires, and upload medical records, etc. This is the most secure method and we encourage our patients to participate in Electronic Health Records with Hawaii Naturopathic through CharmPHR. If you are able to do this great, if you feel challenged by technology or computers, we do not wish to add further stress for you, simply FILL IN THE FORM below and request that the forms be emailed to you or arrange to fill them in there at our office. Please complete this form and we will create an account for you.

Once your CharmPHR account has been created, we will send you the necessary forms to complete through the portal. If you are going to print and complete it, we will email the forms to you.

We require that all forms be completed and any medical records gathered and submitted before your appointment. 

If applicable – gather any medical records (recent lab work, testing results, doctors notes, diagnosis, etc.) Download a Medical Records Release Authorization Form, if you need one. Please upload your medical records to your patient portal account, or fax them to 1-844-965-9821.
